Unanswered questions are tracked separately from None. Cluster count is the primary Shoemaker-style screen; severity is provided only for tracking and is not diagnostic scoring.

How scoring works

2Positive cluster = at least one present symptom in that cluster.

3Positive clusters are the primary Shoemaker-style screen.

4Positive symptom count is secondary and describes symptom burden.

5Severity values are tracking-only and are not diagnostic.
